Patient Records
The patient’s chart must contain the following items signed by the patient prior to treatment:
- Patient Consent for the Purposes of Treatment, Payment and Health Care
- Acknowledgement of Privacy Policies and Acknowledgment of Receipt of Clinic Policies form
- Arbitration Agreement form
- Acupuncture Informed Consent form
