Directions
- Indicate a record of each exam performed or observed by a “hash” mark in the appropriate procedure area.
- Add up the number of exams in performed and observed
- Student actively performed 90% of the positioning and participated in technique selection
- This does not include just assisting a technologist in transferring patients
- The clinical instructor must sign the form and indicate a response to productivity and initiative
